Facilities Plan
A draft facilities· plan must be submitted by June 1, and a complete SRF facilities plan must be submitted by August 1 of the calendar year for which the Request for. Ranking was submitted. All facilities plans must contain that described in the EPA publication Handbook of Procedures, Chapter IV, including all updates and anything required by Office of Pollution Control personnel. A SRF Facilities Plan Checklist is very helpful. Generally, as follows: ( # final copies of the Facilities Plan and the Note # and who got a copy - keep on the left-hand side of the Town File) ALSO NUMBER COPIES OF FACILITY PLAN!!!

- Public Hearing (Must be advertised before 6/1) - minimum of one required, to be advertised in the Jackson Clarion-Ledger and an appropriate local newspaper at least 30 days before hearing. One public hearing is required when alternatives have been determined, but before a chosen alternative has been selected. A public hearing is required before the facilities plan has been fully adopted. 1st May

Only send the SRF and new collection line projects to the OPC for approval.
First Submittal of The Facity Plan#
The Plan Selection has to be included and the advertisement for public hearing has to be published prior to submitting draft plan.
Exhibits: All but proof of public hearing and the resolution Appendices: Will include all
September - OPC will publish notice in the paper or Categorical Exclusion (Only Facility Plan) - Needs no further Environmental review
After we know that the project will be funded -- Send the final draft to the OPC - send information to the State Clearinghouse (SRF Loan Application Form, Form 424, 3 exhibits) from the corps, wildlife and archives - prior to doing SRF Application.
Send three (3) copies of bid package to OPC with cc: to Mayor & Attorney, within 10 days of bid date, consisting of:
- Proof of Publications of Advertisement for Bids
- Certified copy of bid tabulation
- Certified copy of proposal of the successful bidder
- Copy of Bid Security
- Certified copy of Resolution of Conditional Award
- Copy of Project Review & Cost Summary
- Contractor's Construction & Outlay Schedules
- Resume(s) of proposed inspector
- Write Mayor recommending award be made to low bidder
- Contractor's Certification of Debarred Firms
- Owner's Certification of Debarred Firms
- Letter to EEO\MBE Coordinator enclosing EEO documents
MBE documentation from low bidder must be received by us within 10 days. We must send MBE documentation to OPC within 14 days of receiving bids. After approval of MBE/WBE documentation send the bidder, bonds, agreement, etc.
14 days after bid opening - check to see that we did receive a copy of the required EEO documentation that the successful bidder submitted to OPC.
Be sure 3 cc: of the site certificate (if applicable) has been sent to OPC
After approval of bids & money from OPC, set pre-construction conference date. The Final Submittal will be totally complete and will reflect comments from Office of Pollution Control.
